Children & Youth Program Registration 2017-18

Please complete a separate registration for each of your children, from infants to teenagers.  We ask that parents of children in our 11 am Church School please also complete the Parent-Helping in Church School form to volunteer to assist on two separate occasions over the course of the program year. Opportunities to help include the Outdoor Church Offering and Creative Arts Project Help.

Please describe any health conditions that may affect your child's ability to participate in church programs (e.g. allergies, learning or behavioral issues, physical challenges, etc.) Include information about medication, if appropriate.

Do we have permission to photograph or video your child participating in church activities for use in church publications, including the website? No names will be associated with images.
A snack of water or juice and animal crackers, goldfish, cheerios, dried fruit, or cookies is offered during the 11:00 hour to children in the Nursery and Church School classes. Does your child have permission to have snack? (Please indicate food allergies in the Medical Information section above.
Do we have permission to transport your child in a private vehicle or on public transportation as part of Our Whole Lives, Confirmation or Youth Group outings?
